Summary

Bioactive mesh materials supplemented with doxycycline or hyaluronic acid increased collagen deposition but did not improve scar quality for hernia repair in rats. Further research is needed for better patient outcomes.

Background:

  • Recurrent inguinal and incisional hernias stem from impaired wound healing, with altered collagen formation and poor mesh integration contributing to recurrence.
  • Developing bioactive mesh materials is crucial for modifying scar formation and improving patient outcomes after hernia repair.

Purpose of the Study:

  • To evaluate the impact of bioactive agents on collagen deposition and scar quality in mesh implants for hernia repair.
  • To assess whether supplementing mesh materials with specific agents enhances collagen formation and integration.

Main Methods:

  • Mersilene® mesh samples were incubated with doxycycline, TGF-beta 3, zinc-hydrogeneaspartate, ascorbic acid, or hyaluronic acid, and implanted in Wistar rats.
  • Collagen quantity (collagen/protein ratio) and quality (collagen type I/III ratio) were analyzed at 7 and 90 days post-implantation.

Main Results:

  • Supplementation with doxycycline and hyaluronic acid significantly increased the collagen/protein ratio after 90 days compared to the control group.
  • All supplemented groups showed an increased collagen type I/III ratio over time, indicating scar maturation.
  • No significant differences in collagen type I/III ratio were observed between supplemented and control groups at either time point.

Conclusions:

  • Supplemented mesh materials influenced collagen deposition, with doxycycline and hyaluronic acid showing increased collagen quantity.
  • The tested bioactive agents did not lead to improved scar quality, despite their known roles in wound healing.
  • Further investigation into bioactive agents is required to enhance mesh integration and reduce hernia recurrence rates.
