Background:

  • Bowen's disease, a form of squamous cell carcinoma in situ, can progress to invasive squamous cell carcinoma.

Observation:

  • A 72-year-old man with a history of excised invasive squamous cell carcinoma arising in Bowen's disease developed basaloid squamous cell carcinoma metastatic to both lungs.
  • The metastatic carcinoma was morphologically similar to a recurrent, invasive, moderately to poorly differentiated squamous cell carcinoma on his cheek.
  • The patient underwent Mohs' surgery for the recurrent cheek lesion 12 months before lung metastases were detected.

Findings:

  • The patient died of respiratory failure 19 months after diagnosis of metastatic disease.
  • This case illustrates the aggressive potential of invasive squamous cell carcinoma originating from Bowen's disease.
  • Morphological similarity between the primary/recurrent lesion and metastatic disease was noted.

Implications:

  • Complete surgical excision with adequate margins is critical for managing invasive squamous cell carcinoma arising from Bowen's disease.
  • Early detection and aggressive management are essential due to the potential for metastasis.
  • This case underscores the importance of long-term follow-up for patients with a history of Bowen's disease and invasive squamous cell carcinoma.
