Area of Science:

  • Cardiovascular Science
  • Ischemic Heart Disease Research

Background:

  • Preconditioning is a highly effective cardioprotective strategy.
  • It can be triggered by various methods, including brief ischemia, pharmaceuticals, and volatile anesthetics.
  • Remote preconditioning demonstrates that ischemia in one organ can protect the heart.

Purpose of the Study:

  • To review the concepts of preconditioning and postconditioning.
  • To explore the mechanisms underlying these protective effects.
  • To highlight the clinical relevance of preconditioning and postconditioning.

Main Methods:

  • Literature review of preconditioning and postconditioning.
  • Characterization of different preconditioning and postconditioning interventions.
  • Discussion of proposed underlying mechanisms.

Main Results:

  • Preconditioning involves interventions before index ischemia.
  • Postconditioning involves interventions after index ischemia, reducing dysfunction and infarct size.
  • Remote preconditioning and postconditioning involve protection initiated by interventions in distant organs.

Conclusions:

  • Preconditioning and postconditioning are vital cardioprotective strategies.
  • These strategies can be initiated remotely, offering broader therapeutic potential.
  • Understanding their mechanisms is key to clinical application in heart protection.
