Stapled hemorrhoidopexy outcomes improve with a higher staple line. A staple line of 22 mm or more above the dentate line reduces pain medication needs and speeds return to work.

  • Previous research indicated that staple line height and specimen histology predict postoperative pain.

Purpose of the Study:

  • To investigate and refine the predictive role of staple line height in postoperative outcomes following stapled hemorrhoidopexy.
  • To assess the correlation between staple line height and patient recovery metrics.

Main Methods:

  • Prospective data collection from 75 patients with Grade 3 and 4 mixed hemorrhoids undergoing stapled hemorrhoidopexy.
  • Evaluation of preoperative, intraoperative, and postoperative characteristics, including staple line height, histology, complications, return to work, and narcotic use.
  • Statistical analysis using t-test and ANOVA to determine outcome predictors.

Main Results:

  • Staple line height was not a significant predictor of overall postoperative complications.
  • Patients with a staple line height >22 mm experienced significantly shorter durations of narcotic pain management (P=0.024).
  • A higher staple line height ( >20 mm) was inversely related to return to work duration (P=0.01), with a mean return of 9 days compared to 15 days for heights <20 mm.

Conclusions:

  • A hemorrhoidopexy staple line of ≥22 mm above the dentate line is associated with reduced need for postoperative narcotics and an earlier return to work.
  • Staple line distance above the dentate line significantly impacts patient comfort and recovery outcomes.
