Summary

Background:

Carpal tunnel syndrome (CTS) is a common condition affecting the median nerve at the wrist.
Elderly patients present unique considerations for surgical interventions due to potential comorbidities and physiological changes.

Purpose of the Study:

To evaluate the postoperative outcomes of endoscopic carpal tunnel release in patients aged 70 years and older.
To assess the efficacy of this minimally invasive procedure in managing CTS symptoms and physical impairments in the elderly population.

Main Methods:

A cohort of 27 elderly patients (37 hands) diagnosed with CTS underwent endoscopic carpal tunnel release.
Patients were followed up for an average of 35.5 months, with assessments of pain, numbness, and thenar muscle atrophy.

Main Results:

Postoperative pain resolved in all cases.
Complete resolution of numbness occurred in 35.1% of hands; partial improvement was noted in others.
Significant improvement in thenar muscle atrophy was observed, although moderate to severe atrophy persisted in 21.6% of cases.

Conclusions:

Endoscopic carpal tunnel release is a safe and effective option for managing carpal tunnel syndrome in elderly patients.
The procedure demonstrates benefits in pain relief and improvement of muscle atrophy, even in advanced age, though symptom resolution may vary.
