Diuretics and ACE inhibitors are commonly prescribed for heart failure with preserved ejection fraction (HFpEF). Treatment patterns remained consistent one year post-discharge, with age influencing only spironolactone use.
Area of Science:
- Cardiology
- Internal Medicine
- Pharmacology
Background:
- Current guidelines for heart failure (HF) with preserved left ventricular ejection fraction (LVEF) lack specific empirical treatment recommendations.
- The ETICS study aimed to evaluate medical treatment in HF patients with preserved LVEF at discharge and one year post-discharge.
Purpose of the Study:
- To assess the medical treatment patterns for patients hospitalized with their first episode of HF and preserved LVEF.
- To analyze treatment changes from hospital discharge to one year follow-up.
Main Methods:
- Prospective inclusion of 263 consecutive patients (mean LVEF 63+/-8%) hospitalized for a first HF episode.
- Data collection included patient demographics, aetiology (hypertension, ischemic heart disease), comorbidities (atrial fibrillation, diabetes), and detailed medical treatment records at discharge and one year.
Main Results:
- Diuretics were the most frequently prescribed drugs (81% at discharge, 78% at 1 year), followed by ACE inhibitors (49% at discharge, 46% at 1 year).
- Prescription rates and drug dosages remained largely stable between discharge and one year.
- Age did not significantly affect prescription rates, except for spironolactone, which decreased in patients aged 75 years and older at one year.
Conclusions:
- Loop diuretics and ACE inhibitors are the predominant medications for HF with preserved LVEF.
- Further large-scale multicenter trials are necessary to establish standardized background treatments for HFpEF, beyond addressing etiological factors.
