Peptic Ulcer Disease (PUD) is characterized by the development of ulcers in the stomach or duodenal mucosa. Its pathophysiology is complex, involving a balance between damaging and protective elements.

Summary

Chronic leg ulcers in young adults can signal vascular issues. This case highlights elevated homocysteine levels (hyperhomocysteinemia) as a treatable cause of leg ulcers in young individuals.

Background:

Chronic leg ulcers are uncommon in young adults.
Vascular causes like venous incompetence are typically suspected.

Observation:

A 26-year-old man presented with an eight-month history of leg ulcers.
Doppler studies suggested venous incompetence, but ulcer characteristics were atypical for stasis alone.
No signs of collagen vascular disease were present.

Findings:

Serum analysis revealed significantly elevated homocysteine levels, indicating a hyperviscosity state (hyperhomocysteinemia).
Treatment involved vitamin supplementation (B1, B2, B6, B12, folic acid), mecobalamin, trimethylglycine, and targeted antibiotic therapy with povidone-iodine dressings.
