Background:

Clinical decision-support systems (CDSSs) are increasingly vital for improving healthcare quality.
The development of CDSSs has matured over recent decades, driven by the need for better information delivery and integration into healthcare workflows.

Purpose of the Study:

To review literature and identify trends in the development of CDSSs over the past few decades.
To provide insights into future directions for creating successful and usable CDSSs.

Main Methods:

Literature search of PubMed for papers published in the last five years with "Decision Support Systems" in the title.
Inclusion of expert knowledge for earlier developments in the field.

Main Results:

Modern CDSSs aim for seamless integration with healthcare organizational dynamics and knowledge management.
Maturation of knowledge representation, electronic health record (EHR) systems, standard terminologies, and messaging standards facilitates integrated CDSS development.
Methodologies for constructing and evaluating CDSSs have emerged due to the demand for effective and evolving systems.

Conclusions:

Despite numerous approaches for clinical knowledge management, designing and implementing durable, evolving CDSSs remains an active research area.
Advancements in EHR and infrastructure standards are expected to enable significant contributions from CDSS implementers to healthcare delivery.
