Area of Science:

  • Pediatric Surgery
  • Minimally Invasive Surgery
  • Urology

Background:

  • Undescended testis (cryptorchidism) is a common pediatric endocrine and surgical disorder.
  • Laparoscopy is increasingly utilized for the diagnosis and management of impalpable undescended testes.
  • A standardized classification of laparoscopic findings is crucial for guiding treatment strategies.

Observation:

  • A classification system for laparoscopic findings in 117 impalpable testes (95 patients) was developed.
  • Testicular position, size, and viability were assessed post-intervention.
  • Follow-up data over 3 years informed the efficacy of different laparoscopic approaches.

Findings:

  • Six types of laparoscopic findings were identified, guiding management from observation to orchiopexy or orchidectomy.

  • Laparoscopic orchiopexy for testes near the internal ring resulted in viable testes, though some were high-scrotal.
  • Laparoscopically staged Fowler-Stephens orchiopexy for high abdominal testes showed a low rate of atrophy.
  • Implications:

    • This classification system simplifies decision-making for pediatric surgeons managing undescended testes.
    • Laparoscopic orchiopexy is a viable treatment for intra-abdominal testes.
    • Staged laparoscopic Fowler-Stephens orchiopexy is the preferred method for high abdominal testes unsuitable for one-stage repair.
