Background:

High levels of expressed emotion (criticism, hostility, over-involvement) in families are linked to increased relapse rates in schizophrenia.
Psychosocial interventions aim to reduce expressed emotion within families and are widely implemented.

Purpose of the Study:

To evaluate the effectiveness of community-based family psychosocial interventions for schizophrenia compared to standard care.

Main Methods:

Systematic review and meta-analysis of randomized and quasi-randomized controlled trials.
Searches of the Cochrane Schizophrenia Group's Register and trial reference lists.
Data extraction and calculation of relative risk (RR), confidence intervals (CI), and number needed to treat (NNT).

Main Results:

Family intervention may decrease relapse frequency (RR 0.71, NNT 8) and hospital admissions (RR 0.78, NNT 8).
Improved medication compliance (RR 0.74, NNT 7) and reduced expressed emotion were observed.
No significant effect on dropout rates or clear evidence regarding suicide prevention/promotion.

Conclusions:

Current evidence suggests potential benefits of family intervention for schizophrenia, but clinicians cannot be fully confident of the effects.
Further data from completed trials are crucial for informing practice.
Additional trials are warranted if aligned with routine care parameters.
