Summary

Corticosteroids are controversial for treating viral myocarditis. This review found no randomized trials supporting their use, indicating a need for further clinical research on this uncommon heart condition.

Area of Science:

  • Cardiology
  • Immunology
  • Infectious Diseases

Background:

  • Myocarditis is an uncommon heart disease.
  • Autoimmune mechanisms may follow viral infections, causing myocardial inflammation and necrosis.
  • The efficacy and safety of corticosteroids for viral myocarditis remain debated.

Purpose of the Study:

  • To evaluate the benefits and harms of corticosteroid treatment for viral myocarditis.
  • To synthesize evidence from randomized controlled trials (RCTs) on corticosteroid use in viral myocarditis.

Main Methods:

  • Comprehensive literature search of multiple databases (Cochrane Heart Group Register, Cochrane Library, MEDLINE, EMBASE, BIOSIS, LILACS, Chinese Biomedical CD Database) up to April 2004.
  • Included RCTs comparing corticosteroids with placebo, no intervention, or other therapies for viral myocarditis.

Related Experiment Videos

  • Data extraction by two independent reviewers; results presented as risk ratios and weighted mean differences with 95% confidence intervals.
  • Main Results:

    • No randomized controlled trials meeting the inclusion criteria were identified.
    • The search yielded 507 articles, but none were eligible for review.

    Conclusions:

    • There is a lack of randomized evidence to support the use of corticosteroids in treating viral myocarditis.
    • Further high-quality clinical trials are necessary to determine the role of corticosteroids in managing viral myocarditis.
