Background:

  • Thymoma is associated with various autoimmune diseases.
  • Thymectomy does not guarantee remission of these associated disorders.

Observation:

  • A 50-year-old man with thymoma presented with migratory polyarthritis.
  • Post-thymectomy, he developed worsening polyarthritis, hematuria, and azotemia.
  • Diagnosis of microscopic polyangiitis was confirmed via elevated myeloperoxidase-ANCA and renal biopsy.

Findings:

  • Microscopic polyangiitis, previously unassociated with thymoma, was diagnosed in this patient.
  • Treatment with prednisone and cyclophosphamide led to resolution of articular and renal symptoms.
