Summary

Sentinel lymph node (SLN) biopsy is feasible in pregnant patients with breast cancer or melanoma. This limited study found no adverse effects on deliveries, but risks of tracers should be discussed.

Background:

  • Breast cancer and melanoma diagnosis during pregnancy pose unique challenges.
  • Sentinel lymph node (SLN) biopsy is an alternative to lymphadenectomy for these cancers.
  • Limited data exists on SLN biopsy safety and utility in pregnant patients.

Purpose of the Study:

  • To review the experience with SLN mapping in gravid patients.
  • To assess the feasibility and safety of SLN biopsy during pregnancy.

Main Methods:

  • A retrospective review of SLN mapping procedures performed during pregnancy across North Carolina academic institutions.
  • Inclusion of cases from nine pregnant patients undergoing SLN biopsy for breast cancer or melanoma.

Main Results:

  • SLN were identified in all nine cases.
  • Procedures were performed for breast cancer (three) and melanoma (six).
  • No adverse reactions to SLN procedures were reported; one patient developed a seroma. All deliveries were at term without known adverse effects.

Conclusions:

  • SLN mapping procedures appear feasible in pregnant patients.
  • Potential risks of vital dye or radioactive tracers must be weighed against delaying therapy or omitting nodal staging.
  • This limited experience does not constitute a general endorsement but suggests careful consideration is warranted.
