Summary

Management of undescended testis tumors in 21 patients showed excellent survival for early stages (I, IIb) with radiotherapy or node dissection. Advanced stages (IIc, III) benefited from chemotherapy, demonstrating salvageability.

Background:

  • Undescended testis (cryptorchidism) is associated with an increased risk of testicular tumors.
  • Management strategies for testicular tumors in undescended testes require careful consideration due to anatomical challenges.

Purpose of the Study:

  • To report the experience in managing testicular tumors occurring in undescended testes.
  • To evaluate the outcomes of different treatment modalities based on clinical stage.

Main Methods:

  • Retrospective review of 21 patients with testicular tumors in undescended testes.
  • Clinical staging, histopathological analysis (seminoma, non-seminoma, mixed tumors), and treatment protocols (radiotherapy, retroperitoneal node dissection, induction chemotherapy VAB-6).

Main Results:

  • Tumors were predominantly in the inguinal testis (15/21) and unilateral (17/21).
  • Early-stage disease (I, IIb) achieved 100% 3- and 5-year survival with radiotherapy or node dissection.
  • Advanced stages (IIc, III) showed complete response in 45% and partial response in 55% with induction chemotherapy, with 3-year survival at 45% and 5-year survival at 33%.

Conclusions:

  • Early-stage testicular tumors in undescended testes have excellent prognoses with current treatment protocols.
  • Advanced-stage disease can be salvaged using a multimodal approach including surgery, chemotherapy, and radiotherapy.
  • Effective management of these rare tumors requires tailored treatment strategies based on stage and histology.
