Summary

Corneal ectasia developed 9 years after photorefractive keratectomy (PRK). Ultrastructural analysis revealed stromal degeneration but normal endothelium in this late complication of laser eye surgery.

  • Corneal ectasia is a rare but serious complication following refractive surgery.

Observation:

  • A case of corneal ectasia presenting 9 years after PRK is described.
  • Clinical examination showed inferior topographic steepening and protrusion of corneal surfaces.
  • Advanced imaging revealed significant thinning of the cornea.

Findings:

  • Transmission electron microscopy (TEM) demonstrated degeneration of the epithelial basement membrane and stromal keratocytes.
  • Apoptotic keratocytes and extracellular matrix debris were observed within the corneal stroma.
  • Corneal endothelial cells remained morphologically normal.
