Summary
Idiopathic granulomatous orchitis is a rare testicular condition. This case report highlights its non-distinguishable presentation from testicular tumors, requiring orchiectomy for diagnosis.

Background:
- Idiopathic granulomatous orchitis (IGO) is an exceptionally rare inflammatory condition affecting the testes.

- A 76-year-old male patient presented with clinical and radiological findings suggestive of a testicular tumor.
- A testicle was surgically removed and sent for pathological examination.

- Morphological findings were consistent with idiopathic granulomatous orchitis.
Implications:
- IGO cannot be reliably differentiated from testicular neoplasms using current clinical or ultrasonographic methods.
- Definitive diagnosis of IGO necessitates surgical intervention, typically orchiectomy.
