Summary

This study found that esomeprazole did not significantly reduce overall post-operative nausea and vomiting (PONV) incidence. However, it did reduce the total amount of vomit, potentially benefiting patients at risk of aspiration.

Background:

Esomeprazole is a proton pump inhibitor (PPI) that decreases gastric acid production.
Post-operative nausea and vomiting (PONV) is a common complication following surgery.
This study investigated esomeprazole's efficacy in preventing PONV.

Purpose of the Study:

To evaluate the effect of peri-operative esomeprazole on the incidence and severity of PONV.
To identify risk factors associated with PONV in surgical patients.

Main Methods:

A double-blind, randomized trial involving 284 patients undergoing gynecological or cholecystectomy surgery.
Patients received either esomeprazole 40 mg or placebo peri-operatively.
A standardized anesthesia regimen was administered to all participants.

Main Results:

No significant difference in the overall 24-hour incidence of PONV between esomeprazole and placebo groups (77% vs. 81%).
Esomeprazole significantly reduced the mean amount of vomit (52g vs. 86g).
Neostigmine, opioid use, and type of surgery were identified as significant PONV risk factors.

Conclusions:

Esomeprazole did not provide a clinically relevant reduction in the overall incidence of PONV.
Esomeprazole significantly decreased the total volume of emesis post-operatively.
This reduction in vomit volume may be beneficial for patients with aspiration risk.
