The increasing shortage of donor kidneys necessitates using non-heart-beating (NHB) donors. While kidneys from NHB donors can be transplanted after prolonged waiting periods, logistical challenges limit utilization.

  • Statistical comparison of non-use rates between different time intervals.

    • Kidney non-use rates were 13% (0-2 hours), 33% (2-5 hours), and 45% (>5 hours).
    • A significant difference in non-use rates was observed between the 0-2 hour and >5 hour groups (P < .05).
    • Despite longer waiting times, 16 transplants used kidneys >2 hours and 12 used kidneys >5 hours.

    Conclusions:

    • Usable kidneys can be obtained from NHB donors even with protracted withdrawal periods.
    • A 5-hour cut-off for agonal period duration presents logistical difficulties.
    • The current 5-hour cut-off results in the non-utilization of approximately one-third of potential donor kidneys.
