Summary
This case report highlights the first instance of osteonecrosis linked to chronic myeloid leukemia (CML) in Nigeria. Early blood tests are crucial for diagnosing CML and preventing severe complications like avascular necrosis.
Area of Science:
- Hematology
- Orthopedic Surgery
- Oncology
Background:
- Osteonecrosis is documented in sickle cell disease but not previously linked to chronic myeloid leukemia (CML) in Nigeria.
- This study presents the first reported case of CML-associated osteonecrosis in Nigeria.
Observation:
- A 15-year-old female presented with hip pain, abdominal swelling, and weight loss.
- Diagnosis revealed chronic myeloid leukemia and avascular necrosis of the right femoral head.
Findings:
- The patient experienced significant gait improvement within two weeks of starting cytoreductive therapy for CML.
- Despite treatment, crutches are still necessary due to extensive bone damage from prolonged leukemic impact.
Implications:
- Osteonecrosis of the femoral head can be a complication of CML due to hyperviscosity-induced leukostasis.
- Routine peripheral blood counts in patients with non-hematological symptoms may prevent severe disease complications.

Osteoclasts in Bone Remodeling
Osteoclasts are cells responsible for bone resorption and remodeling. They originate from hematopoietic progenitor cells present in the bone marrow. Numerous progenitor cells fuse to form multinucleated cells, each with 10-20 nuclei. A single osteoclast has a diameter of 150 to 200 µM. These cells have ruffled borders that break down the underlying bone tissue and release minerals such as calcium into the blood in bone resorption. Osteoclasts cling to bones with their ruffled edges during bone...
Bone Disorders
Aging and its effect on bone remodeling is the most common cause of bone disorders. In young and healthy people, bone deposition and resorption happen at an equal rate to maintain optimal bone health.
Bone deposition is also affected by the levels of sex hormones like estrogen and testosterone that promote osteoblast activity and bone matrix synthesis. When the level of these hormones decreases due to aging, it causes a reduction in bone deposition. As a result, bone resorption by osteoclasts...

Disorders of Leukocytes
Leukocyte disorders can lead to either leukopenia, characterized by an abnormally low leukocyte count, or leukocytosis, marked by a very high leukocyte number.
Leukopenia may result from bone marrow disorders, autoimmune diseases, and infectious diseases. For example, conditions such as multiple myeloma and aplastic anemia can impair the bone marrow's ability to produce adequate leukocytes. Similarly, autoimmune diseases like lupus and viral infections such as HIV can prompt the immune system...

Bone Marrow Sampling and Transplants
Bone marrow transplant is a potential cure for several diseases, including cancer and specific genetic disorders. Notably, this procedure is applicable for patients suffering from aplastic anemia, certain types of leukemia, severe combined immunodeficiency disease (SCID), Hodgkin's disease, non-Hodgkin's lymphoma, multiple myeloma, thalassemia, sickle-cell disease, and certain cancers.
The transplant begins with high doses of chemotherapy and radiation treatment, which aim to destroy the...

Bone Remodeling
Bone remodeling is a continuous and balanced process of bone resorption by osteoclasts and bone formation by osteoblasts. In adults, it helps maintain bone mass and calcium homeostasis. While mechanical stress can stimulate turnover as part of the normal maintenance and reparative process, several hormones also regulate bone remodeling.
Cytotoxic Edema: Pathophysiology
Cytotoxic edema is a form of cerebral edema characterized by intracellular swelling of neurons, astrocytes, and other glial cells. It develops when the mechanisms responsible for maintaining ionic gradients across the cell membrane become impaired. Under normal physiological conditions, the sodium–potassium ATPase actively transports sodium ions out of the cell and potassium ions into the cell, preserving osmotic balance and enabling electrical signaling. This pump requires a continuous supply...
