Background:

Sarcoidosis rarely affects the sinonasal system.
Isolated sinonasal sarcoidosis, without pulmonary disease, is exceptionally uncommon.

Observation:

This study reports on four patients with isolated sinonasal sarcoidosis.
Histological confirmation was found in the nasopharyngeal mucosa in one patient.
None of the patients exhibited pulmonary or other extrapulmonary sarcoidosis manifestations.
