Switching from lamotrigine (LTG) to valproate (VPA) can be complex. Abruptly stopping LTG and rapidly increasing VPA proved safe and effective in epilepsy patients.

Area of Science:

  • Neurology
  • Pharmacology

Background:

  • Lamotrigine (LTG) and valproate (VPA) are antiepileptic drugs.
  • Switching between LTG and VPA can lead to adverse events due to drug interactions.

Observation:

  • Two initial cases showed complications despite a cautious LTG to VPA switch, considering VPA-induced LTG serum increase.
  • Successful outcomes after abrupt LTG withdrawal in these cases prompted further investigation.

Findings:

  • A systematic switch involving abrupt LTG discontinuation and rapid VPA dosage increase was performed in five consecutive patients.
  • This strategy resulted in a satisfying outcome for all patients requiring the drug exchange.

Implications:

  • An abrupt dosage change-over strategy from LTG to VPA is recommended.
