Summary

A rare case of Rapunzel syndrome, a gastric trichobezoar extending into the small intestine, is presented. This diagnosis was confirmed after surgery in an 18-year-old female, marking the first reported instance in the Middle East.

Background:

  • Rapunzel syndrome is a rare condition characterized by a gastric trichobezoar with a prolonged tail extending into the small intestine.
  • It is most commonly associated with trichophagia, a disorder involving compulsive hair pulling and ingestion.

Observation:

  • An 18-year-old female presented with nonspecific gastrointestinal symptoms including anorexia, abdominal pain, and altered bowel habits.
  • Clinical examination revealed anemia, cachexia, depression, and a mobile epigastric mass.
  • Scalp alopecia, endoscopy, and CT scan initially suggested a trichobezoar.

Findings:

  • Surgical exploration via laparotomy, gastrotomy, and enterotomy was required for definitive diagnosis of Rapunzel syndrome.
