Background:

  • Human immunodeficiency virus (HIV) infection can lead to opportunistic retinal infections.
  • Progressive outer retinal necrosis (PORN) is a severe ocular complication in immunocompromised individuals.

Observation:

  • A 38-year-old male with HIV presented with atypical, solitary, yellowish-white macular lesions in both eyes.
  • Ocular examination revealed no vitreous inflammation.

Findings:

  • Optical coherence tomography (OCT) demonstrated areas of extremely low reflectivity in the outer retina with irregular borders.
  • OCT revealed selective necrosis of the outer retinal layers, consistent with PORN.
