Summary

Surgery can trigger tumour lysis syndrome (TLS) in children with high-grade lymphoma. Anesthetists must monitor for TLS during operative procedures in patients with high tumor burdens.

Background:

Observation:

  • An unusual case of TLS occurred in a child with high-grade lymphoma during a staging laparotomy.
  • The patient experienced refractory ventricular arrhythmia requiring intensive resuscitation.
  • Continuous venous-venous hemodialysis was necessary in the intensive care unit.

Findings:

  • Surgical procedures, such as staging laparotomy, may act as a trigger for TLS development.
  • High tumor burden in lymphoma patients increases the risk of TLS, especially perioperatively.
  • Refractory ventricular arrhythmias can be a presenting symptom of TLS in pediatric oncology patients.

Implications:

  • Anesthetists and surgical teams should maintain a high index of suspicion for TLS in pediatric lymphoma patients undergoing surgery.
  • Perioperative monitoring and management strategies for TLS should be considered in high-risk surgical oncology cases.
  • This case highlights the importance of interdisciplinary collaboration in managing complex pediatric oncologic emergencies.
