Summary

Orbital pseudotumor, an inflammatory eye condition, can mimic orbital cellulitis. Prompt diagnosis with imaging and steroid treatment led to symptom resolution in a 33-year-old woman.

  • A 33-year-old woman presented with acute monocular orbital pain, photophobia, and restricted extraocular movements.
  • Initial diagnosis in the emergency department was orbital cellulitis, treated with intravenous antibiotics.

Findings:

  • Computed tomography revealed inflammation of the sclera, optic nerve, muscle, and orbital adipose tissue, consistent with orbital pseudotumor.
  • Symptoms resolved promptly after discontinuing antibiotics and initiating tapering steroid therapy.

Implications:

  • Orbital pseudotumor should be considered in the differential diagnosis of acute orbital inflammation presenting to emergency departments.
