Summary

A 47-year-old woman presented with an incidentally discovered mediastinal mass. Further imaging revealed a lymphangioma extending to the thoracic inlet, with a similar cyst found in the breast.

The development of lymphatic tissues and vessels in embryonic life begins around the fifth week. These structures originate from the mesoderm layer, with lymph sacs emerging from developing veins.
The first lymph sacs to form are the paired jugular lymph sacs located at the junction of the internal jugular and subclavian veins. Observation:

  • Incidental finding of mediastinal shadow enlargement on X-ray.
  • Chest computed tomography (CT) revealed an anterior mediastinal mass with high-protein density.
  • Magnetic resonance imaging (MRI) confirmed the lymphangiomatic nature of the lesion.

Findings:

  • A 23x15 mm oval cyst with similar characteristics was identified in the right breast.
  • The mediastinal lymphangioma extended to the superior thoracic aperture.
  • The patient had a prior history of forearm lymphangioma.
