Adventitial cystic disease (ACD) is a rare vascular condition causing leg pain. Surgical removal of popliteal artery cysts, linked to knee joint development, provided successful treatment.

Area of Science:

  • Vascular Surgery
  • Pathology
  • Orthopedic Development

Background:

  • Adventitial cystic disease (ACD) is a rare, non-atherosclerotic condition affecting peripheral arteries, typically the popliteal artery.
  • It is characterized by mucinous cysts within the vessel adventitia, leading to symptoms like intermittent claudication.

Observation:

  • A 30-year-old male presented with left leg intermittent claudication due to a popliteal artery obstructive lesion.
  • Angiography confirmed the lesion, which was successfully treated with surgical excision and graft replacement.

Findings:

  • Pathologic examination revealed adventitial cysts containing acid mucin, compressing and narrowing the popliteal artery lumen.
  • The cysts were lined by synovium-like cells expressing vimentin and CD68, but not cytokeratin.

Related Experiment Videos

Implications:

  • This case suggests ACD may arise from developmental rests of mucin-secreting mesenchymal cells originating from the adjacent knee joint.
  • Successful surgical management highlights the importance of accurate diagnosis and timely intervention for ACD.
