Background:

Acute nonlymphocytic leukemia (ANLL) encompasses a heterogeneous group of myeloid malignancies.
Accurate subclassification of ANLL is crucial for prognosis and treatment selection.
Primitive basophilic differentiation in ANLL presents diagnostic challenges due to overlapping features with lymphoid leukemias.

Observation:

A unique case of ANLL with significant basophilic blast morphology and differentiation is detailed.
Leukemic blasts comprised 83% of peripheral white blood cells and over 90% of bone marrow nucleated cells.
Cytoplasmic vacuoles were observed in some leukemic cells, with varying granule staining characteristics.

Findings:

Cytochemical staining revealed peroxidase-negative granules in leukemic cells, potentially leading to misclassification as lymphoid leukemia.
Positive staining with toluidine blue and characteristic findings with Riu's stain were critical for identifying basophilic differentiation.
The study emphasizes that peroxidase-negative granules in leukemic cells warrant consideration of basophilic differentiation in ANLL.

Implications:

This case underscores the importance of specific cytochemical stains, such as Riu's stain and toluidine blue, in diagnosing ANLL with basophilic features.
Accurate diagnosis of ANLL with basophilic differentiation is essential for appropriate patient management and therapeutic strategies.
