Summary

Lumbar synovial cysts, often mimicking disc herniation, can cause radicular pain. Surgical resection offers definitive treatment with significant functional improvement and low recurrence rates.

Background:

Lumbar synovial cysts are rare, often presenting with symptoms similar to lumbar disc herniation.
Management strategies and diagnostic modalities for these cysts remain subjects of discussion.

Observation:

A retrospective analysis of 6 patients with lumbar synovial cysts revealed unilateral radicular pain.
Magnetic resonance imaging (MRI) was the primary diagnostic tool, with CT and X-rays used in select cases.
All patients underwent surgical cyst resection without arthrodesis.

Findings:

Surgical resection resulted in significant functional improvement in all patients.
Four patients achieved excellent outcomes, and three with motor deficits recovered post-operatively.
No complications or recurrences were reported in this series.

Implications:

Lumbar synovial cysts must be included in the differential diagnosis for radicular compression syndromes.
MRI is the gold standard for diagnosing lumbar synovial cysts.
Surgical resection is an effective and definitive treatment for lumbar synovial cysts, associated with favorable outcomes.
