    Summary

This study introduces a combined dexamethasone and ACTH stimulation test for diagnosing Cushing's syndrome. Adding ACTH stimulation significantly improved diagnostic accuracy compared to dexamethasone alone, making it a recommended screening tool.

    Area of Science:

    • Endocrinology
    • Clinical Diagnostics

    Background:

    • Cushing's syndrome diagnosis requires accurate testing.
    • Dexamethasone suppression tests alone can have overlapping results.

    Purpose of the Study:

    • To evaluate a combined dexamethasone and ACTH stimulation test for Cushing's syndrome diagnosis.
    • To improve the diagnostic validity of screening tests for Cushing's syndrome.

    Main Methods:

    • Administered 1 mg dexamethasone orally at 11 p.m.
    • Administered 25 IU synthetic ACTH intravenously at 8 a.m. the next day.
    • Measured plasma cortisol levels at baseline and after ACTH stimulation in 33 Cushing's syndrome patients and 114 controls.

    Main Results:

    • Dexamethasone alone showed overlapping cortisol values between patients and controls.
    • The addition of ACTH stimulation significantly improved the test's diagnostic validity.
    • The combined test demonstrated superior accuracy in differentiating Cushing's syndrome.

    Conclusions:

    • The combined dexamethasone and ACTH stimulation test is a valuable diagnostic tool.
    • This test is recommended as an effective screening method for Cushing's syndrome in ambulatory settings.
