  • A 27-year-old male with refractory CTCL experienced severe hyponatremia three days post-pentostatin treatment.
  • The patient presented with disorientation and required intensive care for management.

Findings:

  • Clinical and laboratory data suggested both extrarenal sodium loss and syndrome of inappropriate antidiuretic hormone (SIADH).
  • The exact mechanism of pentostatin-induced hyponatremia remains unclear.

Implications:

  • Clinicians must be vigilant for symptomatic hyponatremia in patients undergoing pentostatin treatment.
