Case report: infective endocarditis caused by Brevundimonas vesicularis

This case study highlights Brevundimonas vesicularis as a rare cause of infective endocarditis in an otherwise healthy individual. Effective treatments included ceftriaxone and ciprofloxacin, with ampicillin/sulbactam showing limited efficacy.

Background:

- A case of subacute infective endocarditis in a 40-year-old healthy male is presented.
- The endocarditis was attributed to an occult dental abscess and caused by Brevundimonas vesicularis, confirmed by blood cultures.
Findings:
- The patient successfully recovered without complications following treatment with ceftriaxone and ciprofloxacin.
- Ampicillin/sulbactam treatment failed, and ceftriaxone was discontinued due to an allergic reaction.
Implications:
- This is the first reported case of Brevundimonas vesicularis causing infective endocarditis.
- Third-generation cephalosporins, piperacillin/tazobactam, and ciprofloxacin are suggested as effective treatments for invasive Brevundimonas vesicularis infections.
- Further evaluation is needed for the efficacy of ampicillin/sulbactam against this pathogen.
