    Summary

    A single coronary artery is an extremely rare congenital heart anomaly. This case report details a 65-year-old woman diagnosed with this rare condition, presenting with atypical chest pain.

    • Single coronary artery is a rare congenital anomaly with a very low incidence.
    • It is characterized by the origin of all coronary arteries from a single ostium.

    Observation:

    • A 65-year-old woman presented with atypical chest pain.
    • Diagnostic cardiac catheterization revealed a single left coronary artery.
    • No significant coronary artery lesions were identified.

    Findings:

    • The patient was diagnosed with a single coronary artery anomaly.
    • This anomaly is exceptionally rare, with only twelve previous cases reported in the literature.
    • The absence of coronary lesions in this patient is noteworthy.
