Summary

This case report details a rare isolated dorsal midtarsal dislocation from blunt trauma. Closed reduction was performed, but long-term pain persisted, suggesting open reduction might offer better outcomes for Chopart joint injuries.

Area of Science:

  • Orthopedic Surgery
  • Trauma Surgery
  • Podiatric Medicine

Background:

  • Isolated dorsal midtarsal (Chopart) joint dislocation is a rare injury.
  • Blunt trauma, such as a fall from height, can cause severe foot and ankle injuries.
  • Understanding injury mechanisms and treatment outcomes is crucial for managing complex foot trauma.

Observation:

  • A 45-year-old male sustained an isolated dorsal midtarsal dislocation of the right foot after a fall.
  • Radiographs revealed associated spinal and ankle fractures, but no neurovascular compromise.
  • The midtarsal joint demonstrated dorsal instability with navicular and cuboid displacement.

Findings:

  • Closed reduction and screw fixation were performed for the midtarsal dislocation.
  • At 76-month follow-up, the patient experienced persistent midfoot pain and radiographic changes.

Related Experiment Videos

  • Degenerative changes and dorsal ossification were noted in the midtarsal joint.
  • Implications:

    • This case highlights the challenges in managing isolated Chopart joint dislocations.
    • Open reduction with ligamentous repair, particularly the spring ligament, may yield superior long-term results.
    • Further research into optimal surgical techniques for midtarsal injuries is warranted.
