Summary

Administering high-molecular-weight dextran 70 (H) before surgery did not reduce postoperative adhesions compared to administering it after. This study found no significant difference in adhesion formation or reformation between the two timing methods for H.

Purpose of the Study:

  • To evaluate if administering high-molecular-weight dextran 70 (H) before surgery reduces adhesion formation and reformation compared to postoperative administration.
  • To compare the efficacy of pre- versus post-surgical instillation of H in preventing adhesions.
  • To assess the impact of H timing on adhesion scores after initial injury and subsequent lysis.

Main Methods:

  • Twenty rabbits underwent a standardized surgical injury to the ovary, uterine horn, and peritoneum.

  • Adhesions were blindly scored and lysed after 3 weeks, followed by a second scoring 3 weeks after lysis.

    Conclusions:

    • Preoperative instillation of high-molecular-weight dextran 70 (H) does not provide an advantage over postoperative instillation in preventing adhesion formation or reformation.
    • The timing of H administration is not a critical factor in mitigating surgical adhesions in this rabbit model.
    • Further research may be needed to explore alternative strategies or optimal conditions for using H in adhesion prevention.
