  • Group I: Single-bundle, single-incision transtibial ACL reconstruction.
  • Groups II & III: Double-bundle reconstruction using single-incision transtibial or double-incision outside-in techniques, respectively.
  • Evaluations included subjective IKDC, KOOS, objective IKDC, and KT-1000 measurements at minimum 2-year follow-up.
  • Main Results:

    • KT-1000 side-to-side difference was 2.4 mm (Group I), 1.6 mm (Group II), and 1.4 mm (Group III).
    • The double-bundle, double-incision outside-in technique (Group III) showed improved anteroposterior stability.
    • Fewer patients in Group III experienced a positive pivot shift compared to Group I.

    Conclusions:

    • Double-bundle ACL reconstruction, particularly the double-incision outside-in technique, offers superior anteroposterior stability.
    • This technique effectively reduces residual pivot shift, enhancing functional outcomes after ACL reconstruction.
    • The double-incision outside-in method may provide a more anatomical and functional restoration of the ACL.
