Summary

Straight pedicle screw insertion provides superior stability for lumbar spinal fusion compared to angled insertion. This technique offers more predictable outcomes in posterior lumbar surgery, especially for obese patients.

Area of Science:

  • Orthopedic Surgery
  • Spinal Biomechanics
  • Surgical Techniques

Background:

  • Pedicle screw fixation is the gold standard for lumbar spinal fusion stabilization.
  • Obese patients present challenges for pedicle screw insertion due to soft tissue depth.
  • Angled screw insertion is often necessary but its biomechanical stability is debated.

Purpose of the Study:

  • To compare the biomechanical stability of straight versus angled pedicle screw insertion under cyclic loading.
  • To determine if straight screw insertion offers superior construct stability in lumbar spinal fusion.

Main Methods:

  • 12 lumbar vertebrae (L3-L5) from fresh frozen cadavers were used.
  • Schantz screws were inserted axially (straight) and sagittally (angled) into each pedicle.

Related Experiment Videos

  • Screws were subjected to caudal cyclic loading (50 N for 2000 cycles at 2 Hz) to assess fatigue damage.
  • Main Results:

    • Straight screws exhibited significantly less total fatigue damage (0.398+/-0.38 mm) compared to angled screws (0.689+/-0.96 mm).
    • The straight-screw construct demonstrated statistically significant superior stability (P<0.03).
    • Angled screw insertion resulted in more variable damage, indicating less predictable outcomes.

    Conclusions:

    • Straight pedicle screw insertion yields a more stable construct for lumbar spinal fusion.
    • The angled insertion technique leads to less predictable fixation outcomes.
    • This study validates implanting pedicle screws parallel to the mid-sagittal line for improved stability in posterior lumbar surgery.
