A Novel Approach to Monitoring Graft Neovascularization in the Human Gingiva

  • This study prospectively investigates complication frequencies and predictors in grafting procedures.

Purpose of the Study:

  • Compare complication rates between free soft tissue grafting (FSTG) and subepithelial connective tissue grafting (SCTG).
  • Evaluate acellular dermal matrix (ADM) as an alternative donor material.
  • Identify predictors for postoperative complications.

Main Methods:

  • Prospective study of 75 FSTG and 256 SCTG procedures in 228 patients.
  • ADM used in 5 FSTG and 84 SCTG procedures.
  • Data collected on procedure duration, smoking, gender, age, and postoperative pain, swelling, and bleeding via questionnaire; analyzed with chi2 and logistic regression.

Main Results:

  • Longer surgical duration correlated with increased postoperative pain and swelling (P=0.001).
  • Current smokers had 3x higher risk of swelling (P=0.01).
  • FSTG procedures showed 3x higher risk of pain (P=0.002) and bleeding (P=0.03) compared to SCTG.
  • ADM use significantly reduced swelling (OR=0.46, P=0.02) and bleeding (OR=0.3, P=0.001).

Conclusions:

  • Longer procedures and smoking increase complications after gingival augmentation.
  • FSTG procedures have higher risks of pain and bleeding than SCTG.
  • ADM application significantly reduces swelling and bleeding probabilities.
