Background:

  • Medically unexplained neurological symptoms frequently present with abnormal gait.
  • Previous research has not comprehensively analyzed gait characteristics across diverse psychogenic movement disorders (PMDs) or compared PMDs with and without gait disturbances.

Purpose of the Study:

  • To investigate and compare gait features in patients with PMDs, differentiating between those with normal and abnormal gaits.
  • To analyze differences between PMD patients with co-occurring gait disorders and those with pure psychogenic gait disorder (PGD).

Main Methods:

  • A series of 279 PMD patients were analyzed, categorized into groups with normal (Group I) and abnormal (Group II) gaits.
  • Group II was further divided into those with PMD and additional abnormal gait (II-1) and those with pure PGD (II-2).

Main Results:

  • Excessive slowness of movement was significantly more prevalent in PMD patients with abnormal gaits (Group II) compared to those with normal gaits (Group I).
  • Gait slowness was the predominant feature in patients with PMD and PGD (II-1).
  • Buckling of the knee was the most frequent pattern in pure PGD (II-2), followed by astasia-abasia.

Conclusions:

  • Abnormal gait is a common feature in PMD, with excessive slowness being a key indicator in affected individuals.
  • Specific gait patterns, like knee buckling and astasia-abasia, characterize pure psychogenic gait disorders.
