Background:

  • Squamous cell carcinoma (SCC) within a Zenker diverticulum is exceptionally rare.
  • The patient presented with a functionally inoperable Zenker carcinoma, necessitating a multidisciplinary treatment approach.

Observation:

  • Initial treatment involved neoadjuvant radiochemotherapy followed by cardiac bypass surgery.
  • The patient experienced a complicated postoperative course including cardiogenic shock and myocardial infarction.

Findings:

  • Despite initial inoperability and complications, re-evaluation confirmed the patient's suitability for surgical intervention.
  • A partial hypopharyngectomy, cervical esophageal resection, and lymphadenectomy were performed.
  • Gastrointestinal continuity was restored using a free small bowel graft with microvascular anastomosis.
  • A minor anastomotic leak occurred postoperatively but was managed conservatively.

Implications:

  • This case demonstrates the feasibility of a neoadjuvant radiochemotherapy approach for initially inoperable Zenker diverticulum SCC.
  • Successful surgical management and reconstruction, even after significant perioperative complications, are achievable.
  • Highlights the importance of re-evaluation and adaptive treatment strategies in complex oncological cases.
