This case study highlights a rare instance of isolated cecal necrosis in an elderly woman, successfully treated with laparoscopic partial cecectomy. The minimally invasive approach offers a novel management option for this uncommon condition.

Background:

  • Acute colonic ischemia is a frequent cause of colitis in the elderly.
  • Isolated ischemic necrosis of the cecum is a rare condition, often linked to other health issues.

Observation:

  • A 73-year-old woman with hypertension presented with right lower quadrant abdominal pain, tenderness, guarding, and rebound tenderness, initially suspected as acute appendicitis.
  • Laparoscopic surgery revealed partial necrosis of the cecum, not acute appendicitis.

Findings:

  • The patient underwent laparoscopic partial cecal resection and appendectomy.
  • Pathology confirmed cecal wall necrosis due to vascular thrombosis.
  • This represents the first reported case of partial cecal necrosis managed laparoscopically with only partial resection.

Implications:

  • Laparoscopic partial cecectomy is a viable and potentially less invasive treatment for isolated cecal necrosis.
  • Early diagnosis and surgical intervention are crucial for favorable outcomes in rare cases of cecal ischemia.
  • This case expands the understanding of cecal necrosis management, particularly in elderly patients.
