Summary

Lumbar hernias are rare, often occurring in men. A novel surgical approach combining the Dowd technique with prosthetic mesh successfully repaired a complex case after other methods failed.

Background:

Lumbar hernias, particularly in Grynfeltt's triangle, are uncommon and present unique surgical challenges.
Acquired lumbar hernias often result from trauma, including iliac crest bone harvest and road accidents.

Observation:

The case involved a post-traumatic inferior lumbar hernia where initial laparoscopic and subsequent laparotomy repairs were unsuccessful.
This highlights the complexity and potential for recurrence in challenging lumbar hernia cases.

Findings:

A novel lumbotomic surgical approach, integrating the established Dowd technique with prosthetic mesh, proved effective in this complex case.
The patient remained recurrence-free at a 3-month follow-up, indicating the success of the novel approach.
