Multifocal osteosarcoma, a rare cancer affecting multiple bone sites without lung metastasis, presents a diagnostic challenge. This case report details a successful treatment outcome in a young adult patient.

Background:

  • Multifocal osteosarcoma is defined as tumor occurrence at two or more sites without pulmonary metastases.
  • It can manifest as either synchronous (at the same time) or metachronous (at different times) disease.
  • This rare condition is associated with a generally poor prognosis.

Observation:

  • A 21-year-old male presented with a swollen, painful right knee without a history of trauma.
  • Imaging revealed high-grade osteosarcoma in both the distal tibia and proximal femur.
  • The patient had no evidence of pulmonary metastases at diagnosis.

Findings:

  • Surgical resection and prosthetic replacement of the affected distal femur and proximal tibia were performed.
