    Summary

Extra support significantly increases breastfeeding duration for mothers. Professional and lay support combined showed the most significant impact, particularly for exclusive breastfeeding, though maternal satisfaction requires further study.

    Area of Science:

    • Public Health
    • Maternal and Child Health
    • Evidence-Based Practice

    Background:

    • Breastfeeding offers significant health benefits to both infants and mothers.
    • The World Health Organization (WHO) recommends exclusive breastfeeding for the first six months of life.
    • Despite recommendations, breastfeeding rates in developed countries remain suboptimal.

    Purpose of the Study:

    • To evaluate the effectiveness of various support interventions for breastfeeding mothers.
    • To determine the impact of additional support on breastfeeding duration and exclusivity.

    Main Methods:

    • Systematic review and meta-analysis of randomized and quasi-randomized controlled trials.
    • Searched multiple databases including Cochrane, MEDLINE, EMBASE, and MIDIRS.
    • Included 34 trials involving 29,385 mother-infant pairs from 14 countries.

    Main Results:

    • Combined support interventions increased the duration of any breastfeeding (RR 0.91, 95% CI 0.86 to 0.96).
    • Support interventions had a greater effect on exclusive breastfeeding duration (RR 0.81, 95% CI 0.74 to 0.89).
    • WHO/UNICEF training courses effectively prolonged exclusive breastfeeding (RR 0.69, 95% CI 0.52 to 0.91).

    Conclusions:

    • Additional professional support effectively prolonged any breastfeeding, but effects on exclusive breastfeeding were less clear.
    • Lay support was effective for exclusive breastfeeding, while its impact on overall breastfeeding duration was uncertain.
    • Further research is needed on cost-effectiveness, support in low-initiation settings, and reporting women's views.
