Area of Science:

  • Hepatology
  • Surgical Oncology
  • Gastroenterology

Background:

  • Benign liver tumors, including hemangioma, focal nodular hyperplasia, and hepatic adenoma, vary in prevalence and prognosis.
  • Hepatic adenoma can lead to complications like spontaneous rupture and malignant transformation.
  • Elective surgery for benign liver tumors is debated, with indications including diagnostic uncertainty, symptoms, and complication prevention.

Purpose of the Study:

  • To evaluate the efficacy and safety of elective surgical resection for benign liver tumors.
  • To identify and synthesize evidence from randomized clinical trials comparing elective surgery with non-surgical management.

Main Methods:

  • Comprehensive literature search of multiple databases (Cochrane Hepato-Biliary Group, CENTRAL, MEDLINE, EMBASE, CancerLit, SCI-EXPANDED) up to December 2005.

  • Inclusion of major hepatological and surgical congress proceedings and reference list reviews.
  • Focus on randomized clinical trials in adult patients with benign liver tumors without emergency indications, comparing elective resection to no intervention or sham surgery.
  • Main Results:

    • No randomized clinical trials were identified that met the inclusion criteria.
    • Existing case series were small and heterogeneous, with reported mortality rates ranging from 1% to 17% in studies that reported mortality.

    Conclusions:

    • There is insufficient evidence to support or refute the use of elective surgery for benign liver tumors.
    • High-quality, large-scale, long-term randomized clinical trials are required to establish the benefits and risks of elective surgery.
