An abnormal loop of the extracranial vertebral artery (VA) caused C5 radiculopathy in a 68-year-old woman. Microvascular decompression successfully treated this rare condition.

Area of Science:

  • Neurosurgery
  • Vascular Neurology
  • Radiology

Background:

  • Extracranial vertebral artery (VA) abnormalities are uncommon causes of cervical radiculopathy.
  • Cervical radiculopathy can present with significant neurological deficits, impacting quality of life.

Observation:

  • A 68-year-old woman presented with C5 radiculopathy attributed to an enlarged intervertebral foramen at C4-5.
  • Imaging revealed an abnormal loop of the extracranial VA compressing the C5 nerve root.

Findings:

  • Contrast-enhanced CT and MR angiography confirmed the diagnosis.
  • Initial conservative management failed to alleviate symptoms.

Implications:

  • Microvascular decompression via an anterior approach offers a curative solution for VA loop-induced radiculopathy.
