Summary

Freshly collected platelet concentrates from the AS-104 separator demonstrated superior platelet survival compared to the CS-3000 in patients with bone marrow aplasia/hypoplasia, potentially extending transfusion intervals.

Area of Science:

  • Hematology
  • Transfusion Medicine

Background:

  • Bone marrow aplasia/hypoplasia necessitates regular platelet transfusions.
  • Platelet concentrate (PC) quality and survival impact transfusion efficacy.
  • Different apheresis systems (AS-104, CS-3000) may yield PCs with varying characteristics.

Purpose of the Study:

  • To compare the in vivo survival and recovery of fresh HLA-selected platelet concentrates from AS-104 and CS-3000 apheresis systems.
  • To evaluate the influence of storage bag material and geometry on PC viability.

Main Methods:

  • 115 patients with bone marrow aplasia/hypoplasia received 567 transfusions of fresh HLA-selected platelet concentrates.
  • Platelet counts were monitored daily for up to seven days post-transfusion to calculate post-transfusional increments.

Related Experiment Videos

  • In vitro studies and limited evaluation of AS-104 PCs stored in teflon bags were also conducted.
  • Main Results:

    • Platelets from the AS-104 showed comparable day-one recovery but significantly better survival than those from the CS-3000.
    • These findings align with previous in vitro studies indicating better function and morphology for AS-104 derived platelets.
    • AS-104 PCs stored in teflon bags showed unfavorable results compared to CS-3000 PCs in polyolefine bags, attributed to bag geometry.

    Conclusions:

    • The AS-104 apheresis system yields platelet concentrates with improved in vivo survival, suggesting potential for prolonged transfusion intervals.
    • Storage bag geometry significantly impacts platelet viability, necessitating optimization for extended storage.
