  • Small MLC segments in IMRT plans can amplify the impact of leaf positioning errors on dose accuracy.

Purpose of the Study:

  • To investigate the therapeutic consequences of systematic leaf positioning errors in head and neck IMRT treatments.
  • To quantify the impact of varying leaf pair offset errors on dose discrepancies and treatment plan acceptability.

Main Methods:

  • Four head and neck IMRT treatment plans were modified with systematic leaf pair offset errors ranging from +/-2.0 mm.
  • Thirty-six films were irradiated using original and modified IMRT segments.
  • Dose differences and gamma index (2%/2 mm criteria) were analyzed to evaluate discrepancies.

Main Results:

  • Median dose differences showed a linear relationship with simulated leaf pair errors.
  • A 2.0 mm leaf error resulted in a maximum median dose difference of 1.5%.
  • Gamma analysis indicated that two of the 32 modified plans were unacceptable.
