Aneurysm IV: Nursing Management

Vigilant monitoring for aneurysm rupture is essential for patients undergoing aortic surgery.Preoperative Nursing ManagementContinuously monitor the patient for manifestations of aneurysm rupture, such as pallor, weakness, tachycardia, hypotension, abdominal, back, groin, or periumbilical pain, changes in consciousness, and a pulsating abdominal mass. Regularly assess the patient's peripheral pulses.Instruct the patient to consume a clear liquid diet the day before surgery and administer...

Bacterial Meningitis I: Introduction

Bacterial meningitis is a severe, life-threatening inflammation of the meninges, particularly the pia mater and arachnoid mater, affecting the subarachnoid space, ventricles, and cerebrospinal fluid (CSF). If untreated, it can lead to significant neurological complications or death.Causative AgentsCommon pathogens vary with age and immune status. In adults, major organisms include Streptococcus pneumoniae, Neisseria meningitidis, and Haemophilus influenzae. Streptococcus agalactiae (group B...

Summary

Laminaria placement during abortion, though rarely suspected, can lead to serious infection (sepsis). Prompt antibiotic treatment is crucial for managing this risk in patients undergoing cervical dilatation.

Area of Science:

  • Reproductive Medicine
  • Infectious Diseases
  • Gynecology

Background:

  • Laminaria use for cervical dilatation in artificial abortion is common.
  • Postabortal sepsis is a known complication, but its association with laminaria is infrequently recognized.

Observation:

  • A case of severe sepsis (Enterobacter cloacae) occurred in a nulliparous woman post-artificial abortion with laminaria placement.
  • The patient presented with fever, hypotension, and signs of infection.

Findings:

  • Blood, placenta, and arterial line cultures confirmed Enterobacter cloacae infection.
  • The patient recovered with broad-spectrum antibiotics and supportive care.

Implications:

  • Laminaria placement poses an infection risk due to potential ascending colonization from vaginal/cervical microflora.
  • Preventive measures include surgical disinfection, prophylactic antibiotics, and minimizing laminaria duration.
  • Early recognition and aggressive management of suspected infections are vital.
