Selective electrical stimulation of the hypoglossal nerve improves upper airway stability in obstructive sleep apnea (OSA) models. This approach offers a promising alternative to non-selective stimulation for treating OSA.

Area of Science:

  • Biomedical Engineering
  • Neuroscience
  • Sleep Medicine

Background:

  • Obstructive sleep apnea (OSA) is a common disorder characterized by upper airway collapse during sleep.
  • Electrical stimulation of the hypoglossal nerve (XII) is a potential treatment for OSA, but conventional methods show limited efficacy.
  • The multi-fascicular nature of the XII nerve allows for selective stimulation, but its functional impact on the upper airway (UAW) remains unclear.

Purpose of the Study:

  • To investigate the functional effects of selective electrical stimulation of the hypoglossal nerve on upper airway stability.
  • To compare the efficacy of selective XII nerve stimulation with conventional stimulation methods in a preclinical model.

Main Methods:

  • Animal experiments were conducted on four beagles.

Related Experiment Videos

  • Critical pressure (Pcrit) during simulated inspiration was measured to assess UAW stability.
  • Whole XII nerve stimulation and selective co-activation of genioglossus (GG), hyoglossus (HG), and styloglossus (SG) muscles were applied.
  • Main Results:

    • Whole XII nerve stimulation significantly lowered Pcrit (-0.9 ± 0.4 cmH2O).
    • Co-activation of GG + HG/SG muscles also significantly lowered Pcrit (-1.18 ± 0.6 cmH2O).
    • Both methods demonstrated improved UAW stability compared to baseline (-0.52 ± 0.32 cmH2O).

    Conclusions:

    • Selective activation of XII nerve branches can achieve UAW dilation and patency.
    • Multi-contact nerve electrodes show potential for effective OSA treatment by mimicking non-selective stimulation outcomes.
    • Targeted hypoglossal nerve stimulation offers a promising therapeutic strategy for obstructive sleep apnea.
